How Sildenafil Works
Sexual stimulation causes nitric oxide release in penile tissue, increasing cyclic guanosine monophosphate (cGMP). cGMP relaxes smooth muscle and increases blood flow into the penis.
PDE5 normally breaks down cGMP. Sildenafil inhibits PDE5, allowing this erectile signaling pathway to remain active longer.
Sildenafil therefore supports the natural erectile response. It does not automatically cause an erection and does not directly increase testosterone or libido. Sexual stimulation is still required.
What Do Clinical Studies Show?
Sildenafil has extensive randomized controlled trial evidence supporting its effectiveness for erectile dysfunction.
In the pivotal study by Goldstein and colleagues, published in the New England Journal of Medicine in 1998, 69% of intercourse attempts were successful with sildenafil compared with 22% with placebo during the final four weeks of a flexible-dose trial.
In a systematic review and meta-analysis by Fink and colleagues, approximately 57% of intercourse attempts were successful with sildenafil compared with 21% with placebo. Approximately 83% of sildenafil-treated men experienced at least one successful intercourse attempt compared with 45% receiving placebo.
These findings demonstrate clinically meaningful improvement in erectile function, although individual response varies.

Safety Review Before Sildenafil
Before treatment, the doctor should review cardiovascular disease, blood pressure, diabetes, current medications, alcohol and smoking, previous ED treatment, and hormone-related symptoms.
Sildenafil must not be combined with nitrate medicines, such as nitroglycerin, because the combination can cause a dangerous fall in blood pressure.
